Service Description

Our services ensure that you are enrolled in the proper waiver and other Medicare state plan services; and our Social Work and Advocacy includes educating, planning, advocacy, coordination and monitoring to give you or your loved one access to needed services. We develop Individual Service Plans (ISP) according to your needs and physician requirements. Social Work and Advocacy continually assesses your level of care along with the development, writing, and review of ISP to ensure that you, our client, are receiving the proper level of care.

**These services do not apply to PA waiver participants.
