Service Description
We provide speech therapy directly or by arrangement with a Speech/Language Pathologist under the supervision of a Clinical Supervisor. In accordance with the physician’s plan of care.

Speech Therapy services includes but is not limited to:
- The first visit occurs within 48 hours to determine your immediate care and support needs; and if admitting a clinician for Medicare clients, to determine your eligibility for the Medicare home health benefit, including homebound status, unless otherwise specified by your doctor or requested by you (with permission by your doctor)
- Continual comprehensive assessment to determine level of functioning at appropriate time points
- Continual evaluation and testing of your progress with speech, hearing or language disorders
- Assisting your doctor in determining and recommending appropriate speech as well as hearing services in the development and renewal of your doctor’s Plan of Care
- Providing rehabilitative services for speech, hearing and language disorders
- Recording and reporting your reaction to treatment and any changes in the your condition to your doctor
- Instructing other health team and family members in the home rehabilitation program
- Development, implementation, review and renewal of the Plan of Care and OASIS document in cooperation with the physician and other members of the care team
- Advising and consulting with the family and other members of the care team, as applicable
- Development, implementation, review and revision of the plan of care and documenting clinical and progress notes, summaries, and other clinical record information
- Participation in case conferences, staff meetings, discharge planning activities and in-service programs as indicated
